The Health Trust provides services to Santa Clara County residents with HIV/AIDS and empowers patients and families to solve their day to day needs.
 

Essential services include:

 

Case management for those who meet eligibility criteria to assist with managing medication; communicating with medical and social service providers; negotiating complex systems such as Social Security and health insurance; finding emotional support; mental health and substance abuse treatment referrals; and referrals to other services in the community.

 

Emergency financial assistance

 

Hot meal delivery (for those on case management)

 

Food baskets

 

Homecare (for those on case management)

 

Rental subsidies and support services to help residents find and stay in stable housing

 

Help identifying and receiving benefits including community resources, state and federal resources and medical and wellness resources.
